  vaccinations and medications recommended
• Specific information related to area to be visited – to include risks, precautions, etc.
• Utilization of Travax Encompass program for customized travel information (copies provided)
  

Fees:

• Consultation fee
• Fee for each vaccine or medication – priced separately

Appointment Scheduling:

• Call 798-6611 to schedule appointment (required)
• Appointments are available Monday through Friday – hours may vary
• Provide name, phone number, area to be visited and departure date
• Provide as much time as possible to prepare for possible vaccination series, etc. (recommend minimum of 4 weeks prior to travel)
