TRAVELING?

PLEASE FOLLOW THE STEPS BELOW:

1)      Read all information below. 

2)      Call appointment line @ (910) 798-6611. 

3)      Print and complete a hard copy of the patient questionnaire.  Click HERE to download.

4) Submit your completed questionnaire to the Health Department one (1) week prior to your appointment. 

 Services Provided:

  • Nurse consultation including review of recommended vaccinations and medications for travel to your desired area, such as typhoid, yellow fever, japanese encephalitis, & adult polio vaccinations

  • A customized travel report detailing your travel itinerary which includes travel risks, precautions, diseases, and safety

  • Documentation of vaccines, including required documentation for yellow fever vaccination

  • Services are provided by appointment only.  Appointments are limited.

  • Prior to scheduling an appointment follow the steps below:

    1. On the Internet, go to www.mdtravelhealth.com to determine your vaccine/medication needs for your travel destination.

    2. Review the information on vaccines/medications available at the New Hanover County Health Department by clicking on the applicable links here:
