Maternity Care Coordination

and Child Service Coordination

by one coordinator.

Service begins during pregnancy

for the mother and may continue

for the child after birth.

 

Health information, developmental screenings, counseling, and assistance with resources provided - referrals for transportation, medical care, childcare, financial assistance

Scheduled monthly contacts

and home visits
