The Child Service Coordination Program is a family oriented service for families who have general or specific questions or concerns. It is also for families with children at risk for developmental delays and children with medical/developmental diagnoses or special needs.
Coordinators are Registered Nurses and Social Workers who work with families to meet their unique goal for their child/family. Contacts are scheduled monthly through home visiting. Enrollment does not require long term commitment – closure to the program is by mutual agreement between family and coordinator.
How can Child Service Coordination support children and families?
· Assess developmental screenings
· Offer ways to strengthen parent/child relationships
· Introduce parents to support programs
· Serve as a link between family and medical services
· Identify program, services and resources to meets families unique needs
· Identify family strengths and support goals based on those strengths
