The goals of the Health Check Coordination program are:

• Implement a county-based outreach plan aimed at increasing enrollment in Health Check and Health Choice.

• Increase utilization of EPSDT/Health Check services to assure that Medicaid eligible children receive comprehensive health care.

• Improve the system for coordination of Medicaid services to reduce morbidity and mortality of children in New Hanover County by assuring those eligible children receive appropriate well child exams and recommended follow-up care from public and private health care providers.

Health Choice Insurance for Children

• Started on Oct. 1^st, 1998 for NC children (residents) who are ineligible for Medicaid, but are low income. To apply or request information call: Social Services at 910-341-4700 or Access III of the Lower Cape Fear at 763-0200 ext. 108
